1153 사상체질의 과학적 연구를 위한 논리와 방법 지규용 20241225 ~ 학회지
Despite of registration in the international classification of disease(ICD)-11 chapter 26, Sasang constitutional medicine(SCM) is needed some solutions about scientific research methods and objective constitutional classification yet. For the scientific research, definition and its detailed explanation on each word is presented first, and then a requisite of experimental idea and research hypothesis with additional plurality hypothesis step by step. Then the logical basis of SCM is presented in a deductive way. And the similarity of the phenomena and concepts of constitution with the phenotypic traits, fitness, and embodied cognition modules in evolutionary biology and cognitive neuroscience are tried as a basis of logical extensions. The constitution is defined as following, the unique biological and psychological attributes of the human body as a adaptive response traits to maintain self-preservation and thermo-metabolic stability against external environmental changes. And using this definition and research hypothesis, logical format of the scientific research can be applied for abduction of constitution theory and then diagnostic variables and their measurable indices can be selected from the above natural science sources and clinical laboratory tests, genetic informatics etc.

In Korean medicine, the lungs and kidneys are viewed as interconnected organs essential for fluid metabolism and respiratory function. The lungs are described as the "upper source of water," dispersing body fluids and qi (vital energy) throughout the body, while the kidneys anchor and store qi. Both traditional and modern medicine recognize that dysfunction in one organ can lead to impairment in the other, with numerous cases illustrating concurrent decline in kidney function in pulmonary diseases and vice versa.This review examines and contrasts lung-kidney interactions as conceptualized in Korean and modern medicine, focusing on their roles in respiration, fluid balance, and immune response. We analyze the physiological basis of lung and kidney interdependence, utilizing traditional theories from Korean medicine alongside findings from modern clinical research. In Korean medicine, the lung-kidney relationship is characterized by a mutual dependence that supports homeostasis in both respiration and fluid regulation. Pathological observations, such as those in chronic obstructive pulmonary disease (COPD) and chronic kidney disease (CKD), confirm the modern understanding that these organs share roles in acid-base balance, blood pressure control, and fluid metabolism. Autoimmune diseases, such as systemic lupus erythematosus (SLE) and ANCA-associated vasculitis, also commonly impact both organs, demonstrating immune system-mediated injury in both the lungs and kidneys. Our findings underscore the importance of a combined therapeutic approach that considers both lung and kidney function in treating diseases affecting either organ. Korean medicine’s integrated perspective on lung and kidney interaction aligns with modern medical research, which supports the need for cross-disciplinary treatment strategies. Future research should explore the mechanisms of lung-kidney interaction further to enhance understanding of disease progression and treatment efficacy. This study highlights the physiological and pathological interactions between the lungs and kidneys, presenting an integrated framework that bridges traditional Korean medicine and contemporary clinical insights. Understanding these interdependencies will help develop comprehensive treatment strategies for patients experiencing simultaneous lung and kidney dysfunction.

Research on improving the accuracy of tongue region segmentation in the deep learning process of Korean medicine's tongue diagnosis is actively ongoing. This study aims to propose a segmentation model based on the U-Net using Convolutional Neural Networks (cNN). Binary mask images representing the tongue region were created, and the dataset was randomly divided into an 8:2 ratio for the train and test set. To prevent overfitting, the changes in validation loss were monitored at each epoch. Finally, Dice similarity coefficient (DSC) and Jaccard index (JI) were computed to measure the accuracy of the segmentation model in identifying the target regions. The modified U-Net network showed a significant level of accuracy in tongue region segmentation and minimal overfitting despite a small number of training data sets. The accuracy in the train set reached 0.997 and in the test set the accuracy was 0.993. The Dice similarity coefficient (DSC) score on the test dataset was 0.981 ± 0.017 and the Jaccard index (JI) score was 0.964 ± 0.031. The proposed model based on U-Net for tongue region extraction is anticipated to be effective for practical applications such as computerized tongue analysis systems.

Insomnia is a common health problem experienced in 40% of adults, and there is a tendency to start medical drug treatment in general, but Korean medicine treatment is often combined due to side effects caused by drugs. In this study, a retrospective medical record analysis was performed to observe changes in improvement and drug dosage when collaborative treatment was performed for insomnia patients. A total of 146 patients were surveyed, and 35 cases meeting the inclusion criteria were analyzed. The PSQI(Pittsburgh Sleep Quality Index) of all subjects was significantly reduced from 14.17±3.05 before treatment to 13.34±2.74 after treatment. The number of dosing decreased from 6.36 ± 1.33 to 2.79 ± 2.69 per week. In subjects who received herbal medicine, acupuncture, and cupping treatments, the PSQI score significantly decreased from 14.17±2.74 to 13.29±2.66. It is expected that the combination of korean medical treatment including herbal medicine and medical drug treatment in patients with insomnia can help reduce drug dosage while increasing the therapeutic effect.
