165 고혈압의 침치료에 대한 문헌고찰 : 국내 임상 연구 를 중심으로 정상연, 황여채, 조승연, 이한결, 권승원, 정우상, 문상관, 박정미, 고창남, 박성욱 2022-01 ~ 학회지
■Objectives : This study aimed to collect and analyze clinical studies on the significance of acupuncture in the treatment of hypertension among Korean patients.
■Methods : Among Korean patients with a blood pressure (BP) of 120 mmHg or higher and a diastolic BP of 80 mmHg or higher, those, treated with acupuncture only, were included. A literature search was conducted through 'Embase', 'Medline', 'Science and Technology Information Integration Service (NDSL)', 'Traditional Korean Knowledge Portal (OASIS)', 'PubMed', and 'Scopus'. The search keywords were (hypertension OR 'blood pressure') AND acupuncture. The papers, published before the day of the search (02. January 2022), were included in this study.
■Results : Among the 12 selected papers, seven involved randomized controlled trial (RCT), four be-fore-and-after studies, and one case series. The number of RCTs has increased yearly. The treatment methods used in the studies included needle acupuncture in eight studies, auricular acupuncture in two, pharmacopuncture in one, and si-acupuncture in one. The average num-bers of patients enrolled in RCTs, before-and-after studies, and case series were 21.1, 30, and 23 espectively. Six studies were conducted on prehypertensive patients, two on stage 1 hy-pertension patients, and four on stage 2 hypertension patients. The involved acupoints have been reported to be ST36 in eight papers, LI11 in four papers, and PC6 in three papers. The treatment period lasted for < 1 week in eight studies and 8 weeks in four studies. Acupunc-ture successfully lowered blood pressure in nine out of 12 studies
■Conclusions : Acupuncture is a viable alternative treatment option for prehypertensive patients, who are not taking medications. Additionally, it is also useful in further lowering the BP of patients with stage 1 and stage 2 hypertension in the short term. Large-scale and long-term studies on acupuncture for hypertension should be conducted.

■Abstract : Brady-arrhythmia is defined as an arrhythmia in which the heart rate slows to less than 60 beats per minute. Brady-arrhythmia reduces cardiac output and causes changes in blood flow. As a result, dizziness occurs because not only ischemia occurs in the central nervous sys-tem, but also blood flow in major organs decreases. The insertion of an artificial pacemaker is known as almost the only treatment alternative for patients with brady-arrhythmia with symptoms. This study reports a case of 85 year old male diagnosed with brady-arrhythmia complaining presyncope type of dizziness. The patient underwent complex Korean medi-cine treatments by herbal medicine, acupuncture and moxibustion. The daily average value of pulse rate, K-DHI score, NRS score and the patient's subjective expression of symptoms were used as evaluation tools. This study suggested a significant improvement in symptoms through complex Korean medicine treatments without the use of anti-arrhythmic drugs or insertion of an artificial pacemaker in the patient with brady-arrhythmia complaining dizzi-ness.

163 섬망을 호소하는 비경련성 간질 중첩증 환자에 대 한 복합 한의치험 1례 이다빈, 박호정, 이영선, 조기호, 문상관, 정우상, 권승원, 이한결 2022-01 ~ 학회지
■Abstract : Nonconvulsive Status Epilepticus(NCSE) refers to continuous epileptic condition with al-tered mental status and behavioral symptoms, but without convulsive movement at least 10 minutes. In Salzburg criteria, well known as the diagnostic criteria of NCSE, it is diagnosed with clinical symptoms, electroencephalogram (EEG), and effects before and after the use of antiepileptic drugs (AEDs). Commonly being used to treat NCSE, AEDs are likely to have adverse effects. In the present case, a 85-year-old female NCSE patient complaining de-lirium underwent combined Korean medical treatment by acupuncture and herbal medicine for 13 days. The effect of treatment was assessed with delirium scales including Delirium Rating Scale-Revised-98(DRS-R-98) and Assessment Test for Delirium & Cognitive Impair- ment(4AT) per 2 days. After the treatment, both DRS-R-98 and 4AT scores decreased, and orientation and mental status of patient improved. This case report suggests that Korean medical treatment might be an effective option without side for those NCSE patients com-plaining delirium.

162 동측 반맹을 호소하는 후두엽-두정엽 뇌출혈 환자 에 대한 복합 한의치험 1례 정성훈, 배인후, 김근영, 조기호, 문상관, 정우상, 권승원, 이한결 2022-01 ~ 학회지
■Background : Overall prevalence of visual field loss after stroke was estimated at 31%, ranged from 5.5-57%. Among them, homonymous hemianopia is the most common symptom of post-stroke visual field loss and leads to a decrease in the quality of life and therapeutic effect. However, standard treatment has not been established.
■Case report : A 48-year-old male with right side homonymous hemianopia by a left occipital-parietal lobe hemorrhage was treated with Qu-Ji-Du-Huang-Wan, acupuncture and electroacupuncture for 20 days. We used confrontation visual field exam, automated perimetry and visual analog scale for evaluating symptoms. Following treatment, confrontation visual field exam and the subjective discomfort was improved.
■Conclusions : The present case report suggests that combined Korean medicine treatment might be an ef-
fective treatment of post-stroke homonymous hemianopia. A number of follow up studies should be conducted to clarify the effectiveness of treatment.
