The association of Gastro Esophageal Reflux Disease symptoms and Heart Rate
Variability through a retrospective cross-sectional study

Background/Aims: Gastroesophageal Reflux Disease(GERD) is a disease in which the contents of the stomach flow
back to the esophagus and cause uncomfortable symptoms or complications. The esophagus originates from the
foregut and is stimulated by autonomic nerves through the vagus nerve, and heart rate variability (HRV) is widely
used as a measurement tool to evaluate the function of the autonomic nervous system. Therefore, this study aims
to investigate the relationship between gastroesophageal reflux disease and heart rate variability test results.
Materials and Methods: Among the patients admitted to Weedahm Korean Medical Hospital from November 2021
to March 2023, 168 patients with GERD history were examined for gastroesophageal reflux disease symptoms and
heart rate variability tests to analyze the association.
Results: The average age of all patients was 58.17 years and the average BMI was 22.27. Of a total of 168 GERD
patients, 138 patients had symptoms of reflux, 127 patients with heartburn, 26 patients with globus hystericus, and
138 patients with belching symptoms. People with Gastroesophageal reflux symptoms (reflux, heartburn, globus
hystericus, and belching) had low levels of SDNN, RMSSD, and TP, indicating lower resistance to stress and lower
overall autonomic nervous system activity. SDNN, RMSSD, TP, LF, HF, and LF/HF Ratio all showed lower
tendencies in women than men, but there was no statistical significance.
Conclusions: Through this study, it was confirmed that autonomic nerve function decline is common in GERD
patients. This decline in function may be related to symptoms such as reflux, heartburn, globus hystericus, and
belching, respectively, but there was no statistical significance.

1. 위담한방병원에 2020.3.1~2020.11.20까지 입원한 환자 중 기질적인 소견이 발견되지 않으면서 6개월 이상 지속적인 상복부 팽만감과 조기 포만감, 상복부 쓰림이나 통증, 오심 및 트림 증상을 호소하는 기능성 소화불량 환자군 대상으로 복부경결 변화 연구
2. 기능성 소화불량의 진단 및 경과의 판단에 치료 전후 Algometer의 PPT값의 비교가 그 진단적 가치가 있는지 규명하고, 치료 전후 Algometer의 PPT와 증상개선과의 상관성 비교함.
3. 결론
-기능성 소화불량 환자군 73명에서 명치답답, 더부룩가스, 트림, 역류, 가슴답답, 숨참, 두근불안, 목이물감, 등통증, 두통, 어자러움의 11가지 증상 VAS 값은 유의성
있게 줄어듬.
-복부의 Algometer의 PPT값은 치료의 진행에 따라 점차적으로 유의미하게 커졌으며, 이는 환자의 증상 및 복부의 경직이 치료과정에 따라 줄어든다고 봄.

OBJECTIVES: This research aims to explore the effectiveness of Pugation therapy in managing Phlegm Mass Syndrome by algometer.
METHODS: A study was conducted on 36 patients diagnosed with Phlegm Mass Syndrome at a Korean Medicine Hospitals from May 11, 2020, to December 30, 2021. Measurements were taken using an algometer at specific abdominal points (CV14, CV13, CV12, CV10, CV4, ST25, ST27) on the first day of hospitalization and after Pugation therapy 1day. Additionally, patient symptoms were assessed using the visual analog scale (VAS). Changes in algometer values and VAS scores post-treatment were analyzed using a paired t-test.
RESULTS: Post-treatment, a significant increase in algometer values was observed, indicating reduced abdominal stiffness(Lt. ST25 : 2.60±1.02 → 2.67±1.29). Concurrently, there was a notable decrease in the VAS scores, suggesting an alleviation of patient-reported symptoms(Epigastric discomfort : 5.33±2.94→2.05±2.16/ Feeling bloated : 5.64±3.47→2.66±2.11/ Burping : 4.71±2.88→2.81±2.42/ Reflux : 4.42±3.10→1.31±2.99/ Chest discomfort : 4.59±3.81→2.11±2.66/ Palpitation : 4.33±3.10→1.97±2.82/ Sorethroat : 4.66±3.27→2.93±3.22/ Breathing difficulty : 3.88±3.03→2.20±2.12/ Headache : 4.38±2.44→1.54±1.48/ Dizziness : 3.99±3.33→2.12±2.07/ Back pain : 4.67±2.28→2.85±1.10).
CONCLUSION: The study suggested the potential of Pugation therapy as a viable treatment option for Phlegm Mass Syndrome, marked by significant improvements in both abdominal rigidity and overall patient symptoms.

OBJECTIVES: This study aims to assess the clinical outcomes on the symptomatic characteristics of Damjeok syndrome.
METHODS: A total of 283 patients diagnosed with Damjeok syndrome were evaluated at two Korean Medicine hospitals from April 14, 2020, to October 21, 2023. Algometer pressure measurements were taken at the first point of pain complaint on the CV14, CV13, CV12, CV10, CV4, ST25, and ST27 points on the 1st and 14th days of hospitalization. Additionally, patient symptom severity was measured using the Visual Analog Scale (VAS) on these days. Changes in algometer readings and VAS scores were analyzed using repeated measures t-tests, and a correlation analysis was conducted to examine the relationship between the two.
RESULTS: The study found a significant increase in the threshold of algometer values among the 283 patients. Correspondingly, patient symptom VAS scores significantly decreased as treatment progressed. Notably, a negative correlation was observed between algometer values and patient symptom VAS scores.
CONCLUSION: AThe study suggested that the physical findings of abdominal stiffness, as indicated by algometer readings might be a predictor of symptom severity in the functional dyspepsia patients.
