256 흉통을 호소하는 외상성 늑연골염 환자에 대한 화침을 포함한 한의복합치료 증례 보고 1례 김요환 · 홍예진 · 이재동 · 이승훈 2023-01 ~ 학회지
Objectives : The purpose of this study is to investigate the effect of integrative Korean medicine treatment including fire needling for a patient complaining chest pain due to costochondritis.
Methods : A 83-year-old male inpatient complaining chest pain was treated with acupuncture, fire needling, herbal medicine, and deep friction massage. The patient's symptom was assessed using the Numeric Rating Scale (NRS), Pain Pressure Threshold (PPT), thoracic spine rotation range of motion (ROM), and oral medication intake. The quality of life was assessed using The EuroQoL five-dimensional instrument 3 Level Version (EQ-5D-3L) and EuroQoL Visual Analogue Scale (EQ-VAS). The patient was treated and observed from April 24 to May 13, 2023.
Results : After the treatment, pain NRS and oral medication intake decreased, while PPT and ROM increased. Additionally, the values of EQ-5D-3L and EQ-VAS have shown improvement. No adverse events were observed during treatment.
Conclusions : The results show that integrative Korean Medicine treatment including fire needling could be a viable and safe alternative for chest pain due to costochondritis without fractures. It appears that additional research is required based on long-term intervention.

255 우측 어깨 통증을 호소하는 우측 극상근 건병증 환자에 대한 화타침을 포함한 복합 한방치험 1례 노승진 · 홍예진 · 남동우 · 이재동 2023-01 ~ 학회지
Objectives : The purpose of this report is to demonstrate the therapeutic effects of Hwata acupuncture therapy for a patient who has been complaining of pain and limited range of motion in right shoulder due to right supraspinatus tendinosis.
Methods : A 44-year-old female patient with right shoulder pain was treated with Hwata acupuncture. Symptoms were assessed by Numeral Rating Scale (NRS), Range of Motion(ROM), and Shoulder Pain and Disability Index(SPADI). Treatment was conducted for a total of 10 days from January 29, 2023 to February 9, 2023. Pre-treatment evaluation was conducted on January 28, a day before the treatment session. Intra-treatment and post-treatment evaluation were observed on the 5th and 10th day, respectively.
Results : After treatment, NRS decreased from 4 points to 3 points on the 5th day, and to 2 points on the 10th day. External rotation ROM of the tubercular joint improved from 30 degrees to 45 degrees on the 10th day, while there was no improvements in the 5th day. SPADI scores showed improvements on the 10th day of treatment ; pain scale changed from 76% to 68%, disability scale changed from 66% to 41.25%, and the total score changed from 70% to 51.5%.
Conclusions : The results suggest that integrated Korean medicine treatment including Hwata acupuncture could be an effective therapeutic approach in improving pain and functional disabilities induced by supraspinatus tendinosis.

Objectives : The purpose of this study is to investigate the effect of integrative Korean medicine treatment for a patient complaining left hand phalanges weakness after anterior cervical discectomy and fusion (ACDF).
Methods : A 55-year-old male inpatient was treated with acupuncture, electroacupuncture, pharmacopuncture, herbal medicine and neuromuscular electrical stimulation (NMES). The alleviation of symptoms were assessed using grasp strength, range of motion of phalanges, DASH score (Disabilities of arm, shoulder and hand) and EQ-5D & VAS. The patient was treated and observed from March 14th to March 18th, 2023.
Results : After the treatment, the symptoms showed mild improvement, showing increase of grasp strength and range of motion, and decrease of DASH score.
Conclusions : The results suggest that combined Korean medicine treatment can be a possible option for remaining weakness of left hand phalanges after ACDF. Further study seems to be needed based on long-term intervention.

253 한방병원에 입원한 마약성 진통제 오남용 환자에 대한 다학제적 접근: 증례보고 1례 이지수 · 김민선 · 윤유준 · 이훈구 · 이재동 · 이승훈 · 강중원 2023-01 ~ 학회지
Objectives : This study aims to report the effect and safety of multidisciplinary treatment focused on Korean Medicine (KM) for a patient with opioid analgesic abuse disorders.
Methods : A 36-year-old male inpatient received treatment and was observed from October 6 to October 15, 2022. The KM treatment included herbal medicine, acupuncture, electroacupuncture, and moxibustion. We assessed pain using the numeric rating scale (NRS), the amount of opioid consumption, and the patient's general condition using the insomnia severity index (ISI-K), patient health questionnaire-9 (PHQ-9), beck anxiety inventory (BAI), fatigue severity scale (FSS), and heart rate variability (HRV). Adverse events during the treatment was assessed for the safety evaluation. Department of neuropsychiatry, department of anesthesiology and pain medicine, nutrient support team (NST) shared the patient status.
Results : After the treatment, NRS (average NRS from 9 to 7) and opioid consumption (daily consumption from 6~8 tablets to 4~5 tablets) were decreased and patient's overall condition related to opioid analgesic abuse improved (patient health questionaire score from 17 to 14, beck anxiety index from 42 to 29). Additionally, the multidisciplinary approach during the admission to a KM Hospital received satisfactory feedback from the patient.
Conclusions : The results suggest that combined KM treatment may be a viable option for opioid analgesic abuse disorders. Further studies are warranted, particularly with long-term interventions.
